OPC
poisoning: Organophosphours compounds
are:Parathion,Malathion,Diazinon etc.
Criteria:
1) Binds with cholinesterase enzyme
irreversibly.
2) Strong covalent bond.
3) Highly lipid soluble.
4) Absorbed from skin,lungs,GIT=CNS.
5) Ecothiophate is used in
glaucoma(more polar,so does not go to CNS)
6) Use=Homicidal.
Mechanism:
OPC binds with cholinesterase enzyme by a covalent
bond→Formation of a complex by phosphonilation→initially the complex is
weak→After 2-8 hours the complex is stable→Aging→Irreversible to initial stage.
S/S: Due to increase Ach conc.in the body-
1) GI
tract:Salivation,vomiting,abdominal cramp,diarrhoea,involuntary defaecation.
2) Respiratory
system:Bronchorrhea,broncho-constriction,cough,wheeze,dysponea.
3) CVS:Bradycardia,↓BP.
4) Genito-urinary system:Involuntary
micturition.
5) Skin:Sweating.
6) Skeletal muscle:Weakness,twitching.
7) Nervous system:anxiety,headache,convulsions,
respiratory failure.

Eye:Myosis.
Treatment:
1) Contaminated clothes should be
removed.
2) Skin should be washed.
3) Gastric suction if OPC has been
ingested.
4) Atropine sulphate upto atropinization.
5) Specific
antedote-pralidoxime(Cholinesterase enzyme activator).
6) Administration of cholinesterase
enzyme.
7) Fresh blood transfusion.
8) Fluid-electrolytes balance.
9) Nutrition balance.
